Who is this package for?
New Adult Patients
This package includes:
- An initial one hour visit to discuss your concerns, review your health history, family history, current dietary and lifestyle habits and perform a physical exam. We will discuss whether any lab testing is indicated and come up with an initial plan for you.
- Two 30 minute follow up visits to review any test results and assess your response to treatment while fine tuning your program.
- Personal support via your patient portal to clarify your treatment plan.
Visits may be scheduled in person, via telemedicine (video) or by phone. It is encouraged, although not necessary, for the first visit to occur in person. All package visits must be completed within one year of purchase.

Men’s General Health Package
Who is this package for?
Adult Men
This package includes:
- An initial one hour visit to discuss your concerns, review your health history, family history, current dietary and lifestyle habits and perform a physical exam. Blood will be drawn for screening labs including a CBC, CMP, Lipid Panel, TSH, PSA and Total Testosterone.
- One 30 minute lab review visit to review your test results and create a customized plan.
- Personal support via your patient portal to clarify your treatment plan.
Visits may be scheduled in person, via telemedicine (video) or by phone. It is encouraged, although not necessary, for the first visit to occur in person. All package visits must be completed within one year of purchase.

Children’s Allergy Package
Who is this package for?
Children with any of the following concerns:
